The purpose of this interview is to obtain a comprehensive overview of her health concerns, past
medical conditions, family history, and personal lifestyle factors that may influence her current
health status. This information is crucial for understanding her medical background and

Narrative
Patient PG is a 39-year-old Hispanic female presenting with a chief concern (CC) of low
energy. Her history of present illness (HPI) reveals that she has been experiencing increasing
fatigue for the past two months, along with a weight gain of 10 pounds. She has a past medical
history that includes generalized anxiety disorder, a thyroid nodule, and a partial left
thyroidectomy. She is gravida two and para two.
In her family history (FH), her mother has hypertension (well-controlled with
amlodipine), thyroid disease (with stable levels), and sarcoidosis (without new growth). Her
father passed away at age 62 due to hypertension and heart disease. She has a brother with
obesity and bipolar disorder. Her maternal grandmother had thyroid disease, hypertension,
hypercholesterolemia, and oral cancer (deceased at 82), while her maternal grandfather suffered
from peripheral vascular disease (deceased at 78). Her maternal uncle passed away at age 50 due
to colon cancer. One maternal aunt has hypertension, and another aunt died suddenly at 40 due to
a ruptured aneurysm. Her paternal grandmother had hypertension and passed away at 68 from a
myocardial infarction, and her paternal grandfather died of a myocardial infarction at 54.
