Clinical Manifestation in Mrs. J

The case study shows that Mrs. J had a history of hypertension, CHF and COPD. Her

condition is getting worse, owing to her continued smoking and reluctance to take medications.

The manifested symptoms such as fever, productive cough, nausea, and malaise are preliminary

precursors of the worsening health condition. The patient cannot carry out routine activities like

walking without assistance. Other than the observable features, the patient’s objective data shows

she has a low blood pressure of 90/58, a high heart rate of 118, irregular, high ventricular rate,

and faint PMI. These symptoms indicate respiratory complications that are exacerbating heart

failure and COPD.

Appropriateness of the Nursing Interventions

One of the medications administered is IV Furosemide (Lasix), which is a diuretic. It

reduces the amount of water and salt in the body as it blocks the absorption of sodium and

chloride. It controls the excess fluid in the body, thus reducing blood pressure. Enalapril is an

ACE inhibitor, given to Mrs. J to dilate blood vessels to improve the flow of blood and

oxygenation to the heart. According to Ricci et al. (2016), ACE inhibitors improve the function

of the kidney, heart, and eye and peripheral nerve function. Metoprolol (Lopressor) is a beta-

blocker. It reduces the heart rate and inhibits isoproterenol-induced tachycardia. The case of Mrs.

J. Shows she had a heart rate of 118, yet the normal heart rate of an adult is 60 to 100 beats, an

indication that she is suffering from tachycardia. IV morphine sulfate is an opioid analgesic used

for pain treatment. It helps improve vasodilation, induce respiratory relaxation and thus applied

to Mrs. J to yield a calming effect on her condition. The provision of a short-acting

bronchodilator is a relief for relaxing muscles around the airways to prevent and treat wheezing

and difficult breathing.
