I saw in the Case History Intake Form that Hadley has been
diagnosed with a genetic condition. Please tell me about that.
Yes, Hadley has something called Coffin-Siris syndrome. When she
was very young, she wasn't growing as well as Jack, so because I

have Hashimoto's disease, I asked for a referral to a pediatric
endocrinologist. That doctor diagnosed her with a growth
deficiency and thyroid issues but also referred us for genetic
testing. She was diagnosed with CSS, which helps to explain her
delays in growth and speech.
Areas of Concern
52 seconds
In the Case History Intake Form, you reported speech and
language concerns; are those still your main concerns?
I have two major areas of concern right now. I want to make sure
that Hadley continues to grow, which I feel is being well-addressed
and that we are making progress in that area. I also want to make
sure that she is ready to start kindergarten. My main concern
about that is her speech. If her speech doesn't get better, then her
teacher won't be able to understand her and neither will the other
kids. We can't have a situation where Hadley is dependent on Jack
for her communication with others, either. That won't be good for
either one of them.
54 seconds
Are you concerned about her learning or academic skills?
No, I'm not. Her preschool teacher and the speech therapist at
school tell me she is learning all the things she needs to know
before kindergarten. And the only goals they have on her IEP are
speech goals.
57 seconds
You reported difficulty with specific sounds. Can you please
elaborate?
She has a lot of trouble with a lot of sounds. It seems like she can't
get the consonants at the beginning or ends of words. She tries
really hard but is just not able to make most sounds correctly.

You noted your concerns began when other kids started babbling.
Please tell me more about that.
Well, very early, she had the feeding issues with the tongue
clipping and her growth. We addressed that, then we found out
about the ear wax and she had tubes put in. She didn't babble like
other kids did. Then as she started talking, it was obvious she
needed speech. So really it has been one thing after another since
Hadley was a few months old.

Has Hadley seen any specialists, and does she receive any type of
therapy such as speech, occupational, or physical therapy?
Please refer to the Case History Form.

What are your goals for Hadley?
I want her to grow, and learn, and play, and have friends just like
all of the other kids. We have been working so hard with therapy,
and doctors, and early services to try to get her ready for
kindergarten so she can do all of those things. So far she is a happy
little girl, and I want her experiences to stay positive so that it stays
that way.
