1: Fysiologie van de nieren

1.1: Perfusie: bevloeiing van de nieren

Nier:
- Boonvormig orgaan

- Nefronen: 1 miljoen/nier à ontstaan in cortex

• Visceraal blad dat capillairen omvat
§ Ruimte van Bowman binnenin kapsel
o Proximale tubulus
o Lis van Henle: keert terug naar nierlichaampje
o Macula densa: langs kant van nierlichaampje

- Bloedvaten van de nier:
o A. Renalis (zijtak van de aorta): splitst zich in heel veel ongenoemde takken
o Vele bloedvaten lopen uit in afferente arteriolen: kleine slagadertjes die bloed naar
glomerulaire capillairen aanvoeren
à afferente arteriolen lopen uit in glomerulaire capillairen

Normaal lopen capillairen uit in venen maar hier gebeurt dat niet. Capillairen lopen terug uit in
arteriolen (efferent). De arteriolen voeren bloed weg vanuit de glomerulaire capillairen naar de
peritubulaire capillairen (rond tubuli).

à Afferente arteriolen – glomerulaire capillairen – efferente arteriolen – peritubulaire capillairen

Peritubulaire capillairen monden uit in venen en al die venen komen terecht in v. Renalis en die gaat
draineren in v. Cava Inferior.

DUS:
Het bloed stroomt via de a. Renalis, zijn zijtakken en afferente arteriolen naar de glomerullaire
capillairen. Via efferente arteriolen komt het bloed in peritubulaire capillairen. De venen draineren in
de v. Renalis.

Extracellulair volume:
- Volume van het extracellulair vocht (vocht van buiten cellen)
- 2 compartimenten:
o ¼ plasma in bloedvaten
o ¾ interstitium buiten bloedvaten

(C+V=4/5)

Volume arterieel plasma dat weefsels bevloeit (1/5) = effectief circulerend volume (ECV)
Wij kunnen onmogelijk extracellulair volume detecteren, we hebben geen sensoren voor detectie.
Ons lichaam kan wel het volume arterieel plasma dat de weefsels bevloeit (ECV) detecteren.

EVC vermenigvuldigen met 20 à zo heeft lichaam idee over extracellulair volume

ECV detecteren door: 3 parameters
- Arteriële druk meten: druk in slagaders (= bloeddruk)

~ volume veneus plasma dat terugkeert naar hart
~ hartdebiet (hoeveelheid dat hart pompt)
