ensures client is wearing SHOES.

*assists with use of bedpan* - ANS-PLACES bedpan correctly under client's buttocks.
(pointy end / open end toward the front ... just like a regular toilet seat)

*cleans upper or lower denture* - ANS-*no bold.* think the important parts are don't let
anything touch the surfaces of the sink and re-rinse the denture cup before you put the
dentures back into it.

*counts and records radial pulse* - ANS-After obtaining a pulse by palpating in radial
artery position, records pulse rate within PLUS OR MINUS 4 BEATS of evaluator's
reading. Counts beats for ONE FULL MINUTE. (JUST UNDER THUMB ... between
edge of wrist and tendons). I Really Really like my thumb !

*counts and records respirations* - ANS-records respiration rate within PLUS OR
MINUS 2 BREATHS of evaluator's reading. Count for one full minute. For testing
purposes you may explain to the client that you will be counting the respirations.

*donning and removing PPE (gown and gloves)* - ANS-*no bold.* I'm thinking the really
important parts are NOT to touch the outside of the gown or gloves while you're doing it.

*dresses client with affected (weak) right arm* - ANS-assists to put the right (affected /
weak / frozen) arm through the right sleeve of the top BEFORE placing garment on left
(unaffected / normal) arm. You don't want to move the weak / frozen arm. LEFT arm is
the one you take it off first (since they can move it the easiest) and the LEFT arm is the
one you put it on LAST.

*feeds client who cannot feed self* - ANS-BEFORE feeding client, clien is in an
UPRIGHT SITTING position (45-90 DEGREES).

,*gives modified bed bath (face and one arm, hand and underarm* - ANS-*no bold* I'm
assuming the big one is don't overly expose the patient and move the body gently and
naturally, avoiding force and over-extension of limbs and joints.

*makes an occupied bed (client does not need assistance to turn* - ANS-*no bold* I'm
assuming it's pretty important to make sure you do NOT move the client yourself (since
you're getting into side rail issues) and don't really touch the client (contamination
issues?)

*measures and records blood pressure* - ANS-After obtaining reading using BP cuff
and stethoscope, records both systolic and diastolic pressures each within PLUS OR
MINUS 8 mm of evaluator's reading.

*measures and records urinary output* - ANS-Records contents of container within
PLUS OR MINUS 25 ml/cc of evaluator's reading.

*measures and records weight of ambulatory client* - ANS-Records weight based on
indicator on scale. Weight is within PLUS OR MINUS 2 lbs. of evaluator's reading (If
weight recorded in kg weight is within plus or minus 0.9kg of evaluator's reading).

*performs passive range of motion (prom) for one knee and one ankle* - ANS-While
supporting the limb, moves joints gently, slowly, and smoothly through the range of
motion, discontinuing exercise if client verbalizes pain.

*performs passive range of motion (prom) for one shoulder* - ANS-While supporting the
limb, moves joints gently, slowly, and smoothly through the range of motion,
discontinuing exercise if client verbalizes pain.

*positions on side* - ANS-*no bold.* Probably really important to provide privacy, make
sure head of bed is down and roll as one unit.

*provides catheter care for female* - ANS-While holding catheter near meatus without
tugging, CLEANS at least 4 INCHES of catheter nearest meatus, moving in only one
diretion, away from meatus, using a clean are of the cloth for each stroke. While holding

*transfers from bed to wheelchair using transfer belt* - ANS-before assisting to stand,
LOCK WHEELS on wheelchair.

*washes hands* - ANS-lathers all surfaces of WRISTS, hands and fingers, producing
friction for at least 15 (use 20+) seconds.

5 measurement skills - ANS-BP - blood pressure. Radial pulse. Respirations. Urine
output. Weight.

Abdominal Thrusts - ANS-** ask the resident if he can speak
** call for help
** place hands above the navel and below the ribs

Ambulate Resident Using a Gait Belt - ANS-Supplies: Non-skid footwear, gait belt.
1. Opening and assure nonskid footwear is on.
2. Place gait belt around patient's waist.
3. Ensure a 4 finger space between patient and belt.
4. Have patient place hands on your shoulders while gripping the gait belt with both
arms around patient.
5. Assist patient with standing by counting to three. Let them stand still for a few
seconds and ask if they are dizzy.
6. Walk 10 steps and then 10 back while holding the gait belt with one hand on farthest
side.
7. Frequently ask about comfort, dizziness, or weakness
8. In same position as helping stand, lower patient into chair on count of three.
9. Remove gait belt.

Non slip shoes
Gait belt
Remember to:
Wash hands
Check armband
Explain what you are going to do
Put gait belt on correctly, not grabbing any skin or clothing
Always ask resident how they are doing, do they feel dizzy
Walk at least 10 steps
Support resident by walking on weak side, grasping back of gait belt
Help resident sit down, ask if they are dizzy, remove gait belt, give them call light

Ambulate The Resident Using A Transfer/Gait Belt - ANS-1.) Greet resident, address by
name, and introduce self
2.) Provide explanations to residents about care before beginning and during care
3.)Apply transfer/gait belt before standing resident, placing around th residents waist
and over clothing, secure so that only flat fingers/hand fit under belt, and the belt does
not catch skin or skin folds (e.g. breast tissue)
4.) Provide signal or cue to resident before assisting to stand
5.) Assist resident to stand while holding belt only at the front or only at the nearest side.
(if assisting to stand from the side.)
6.) Ask how resident feels upon standing
7.)Walk resident while standing to the side and slightly behind resident
8.) Provide support while walking resident with an arm around residents back holding
transfer/gait belt
9.) Ask how the resident feels during ambulation
10.) Walk resident at least 10 steps
11.) Assist resident to turn and have back of legs positioned against the seat of chair
before the resident sits.
12.) Provide support to sit resident back into chair
13.) Remove transfer/gait belt from residents waist without harming resident (e.g. pulling
transfer/gait belt.) when seated in chair after ambulation
14.)Maintain own body mechanics when assisting the resident to stand and sit
15.)Leave resident positioned in chair in proper body alignment and hips against back of
seat
16.) Ask resident about preferences during care
17.) Use standard precautions and infection control measures when providing care
18.) Ask resident about comfort or needs during care or before care completed
19.) Promote residents rights and residents safety during care
