Patient Details: Name: Green, Jana Age: 76 Years Gender: Female

Phase 1, Thursday 07:30
You assume care for Mrs. Green, and according to the report you receive from the
off-going nurse, the patient’s respiratory status has improved; however, she had a
restless night spent complaining of pain rated as 3 out of 10 from bone pain.
Hydromorphone PCA is infusing as ordered at 0.2 mg/hour IV continuous mode. You
enter the room and find the patient grimacing and lying on her right side with her
legs drawn up towards her chest. You introduce yourself and ask the patient's
permission to perform your assessment. She agrees to the examination but asks
you to help her get comfortable. The patient states that she has not had a bowel
movement since Sunday and complains of extreme abdominal discomfort.

The patient's breath sounds are diminished throughout upper anterior and posterior
lobes with fine crackles auscultated bilaterally in lower posterior lobes and
respirations labored and shallow. The patient's abdomen is distended and tender to
light palpation with hypoactive bowel sounds in all 4 quadrants. The patient reports
pain in the left lower quadrant as 4 out of 10 with movement and describes the
pain as constant and sharp. An IV 0.9% normal saline is infusing via pump at 150
mL per hour to the patient's left forearm (median vein) with an 18-gauge peripheral
IV with hydromorphone PCA 0.2 mg/hour IV continuous mode per pump as a
secondary infusion at rate ordered.
