Managing Care

Learning Outcomes :
LO1: Critically explore the need to adapt communication skills and develop
therapeutic alliances with people across the life-course in a variety of
settings
LO2: Apply knowledge of patient-centred assessment, care planning and
goal/outcome setting approaches in a coherent and systematic manner
LO3: Demonstrate knowledge of best practice working in partnership with
people, their families/carers, in teams and with the wider multi-disciplinary/
multi-agency network in order to deliver safe and effective person-centred
care
LO4: Source, interpret and synthesise theory, models of care, and robust
research evidence to underpin compassionate, safe and effective nursing
practice relevant to your field of nursing
LO5: Clarify the professional responsibilities and tools used to provide a safe
health care environment, demonstrating an understanding of the legislative
requirements
LO6: Engage in critical self-reflection, sourcing and applying a sound
evidence base to demonstrate awareness of professionalism in the provision
of care

The Nursing Process
• A systematic method

• Holistic assessment- Biopsychosocial model- Engels (1977)
• Includes the Activities of Daily Living (Roper, Logan and Tierney, 1983).

• Assessment: observing, measuring (objective) and communicating
(non-verbal, appearance, cognitive abilities, other forms of
communication needed- subjective data is what the patient says)\
= Create baseline of patient through assessment such as Obs e.g BP,
Temperature etc but also what patient is telling you. Also waterlow assessment
and Must score, mental capacity assessment etc etc.
• Diagnosis: identifying the patients problems/concerns ( taking a
history-time frames, changes)
• Planning: planning care and setting patient-centred goals (realistic)/
aims *goal setting to be explored further later in the module*
= Desirable and achievable for patient
• Implement: being able to apply the nursing actions identified in
planning- interventions completing care plans -specific instructions –
e.g., dressing to be renewed every other day
• Evaluation: determining if the goals set have been achieved and if
needed reassessing.
