D2 – Case Study
Following an experiment in the laboratory a gas tap was left on by mistake, normally
the Senior laboratory technician would check to see if they had all been turned off but
the experiment had run over time and the senior laboratory was in a rush to get
home as they were going to be leaving on a two-week holiday, replacing her was a
trainee laboratory technician that was skilled enough to be left in charge of the
laboratory. Just before the senior laboratory technician left they jotted down a list of
all the task and all the deliveries that would arrive during the fortnight on a scrap
piece of paper leaving it on the laboratory technicians desk. A few moments after the
senior laboratory technician left a cleaner entered the office to clean it and when they
noticed the piece of paper laying on the desk they threw it in the bin with the
previous knowledge that the senior laboratory technician normally kept all important
pieces of paper tucked away in the laboratory notebook.
The next day the trainee laboratory technician arrived to get everything ready that
was needed for that day, the first thing that they did was look for any instructions
that might have been left for them not knowing that they had been thrown out last
night by the cleaner. After they had looked in the laboratory note book they noticed
that there was a pungent smell, believing that it was from yesterday’s experiment
they turned on the air conditioning to get rid of it, not noticing that the smell was off
the gas that was leaking from the taps.
As the trainee laboratory technician, did not receive the note they were not expecting
any deliveries to arrive, but one did it was a delivery of chemicals. As it was getting
dark and the trainee laboratory technicians shift was about to end they decided to
just leave them in the laboratory so that the next person could take over. It was
nearing the end of the shift and another laboratory technician came to start their
shift, so the trainee laboratory technician exited the building to have a cigarette.
Whilst they were outside the other laboratory technician turned on the lights and
slammed the door startling the trainee making them drop their cigarette.
The mix of the cigarette, the chemicals and the leaking gas caused a reaction which
quickly became an explosion but luckily both the laboratory technicians could get
outside and away from the building.
