Signature, printed name and address of each signer and witness. Other parties to instrument and manner in which signer was identified. Government Notary Serves in matters pertaining to gov. No fees or bond. Duties of Hawaii Notary Taking acknowledgments, administering oaths, affirmations or affidavits, taking depositions, noting protests. Civil & Criminal Liability Willfully and knowingly breaching duties, negligent performance resulting in injury. Extortion, Forgery, Pergery, Subornation. Extortion The wresting of anything of value from another by Duress, Menace, Force or Undue exercise of power. Forgery Fraudulent making or altering of a writing with intent to decieve another and prejudice him in some right. Subornation the willful and corrupt procuring of another to commit perjury. Hawaii Permits Notarizing on Sunday and Holidays. Changes Changes must be approved by notary and signing parties. Must be initialed by notary and all parties. Cannot be recorded with out notary initial to changes. affidavit Venue, Body (first duly sworn), Affiant signature, Jurat (subscribed and sworn), Notary signature, Comm exp date and Seal Venue Place of notorial act. EX. State of Hawaii, Cunty of Kauai. Affidavit Sworn statement of affiant. "John Doe, being first duly sworn on oath, deposes and says..." Signature Signature of affiant. Jurat Statement that affiant appeared in person on certain day and took oath and signed affidavit and notary witnessed signature. "Subscribed and sworn to before me on this day....." Notary Signature Signature, Printed Name, Expiration date of Commission, Seal. Acknowledgment Venue, Date of Act, Body (appeared before me), Names, Verify ID " to me known to be", Voluntary Acknowledgment "Acted on free act and deed" ,Testimonium clause "in witness whereof", Notary seal, signature and commission expiration date. Testimonium Clause In witness whereof I have hereunto set my hand and seal. Certificate for every acknowledgment or jurat hall have certification. Certification shall have: Printed notary name, signature, stamp identify jurisdiction, identify type of document, number of pages, date of document, date of certificate.
