Paragraph 1:
In this coursework I will be explaining why meeting the needs of the individuals
requires the involvement of different agencies. I will be researching the roles of
professionals and how they work together to provide the care and support
necessary to meet individuals’ needs.
Paragraph 2:
CCG stands for Clinical Commissioning Group. It is a group of general practices that
come together in each area to delegate the best service to patients and the
population. CCG was founded in April 2013 by NHS England. The role of the
CCG is to assess the health needs of the area. In addition, it orders most NHS
hospitals and communities in their area. This also includes rehabilitation assistance,
emergency and emergency care, education services for people with disabilities, and
mental health services. NHS England is monitoring it. CCG members are the
practice of general practitioners and other health professionals as nurses. This
service can be used for Nusrat Patel. This is because she suffers from learning
