REQUIRES THE INVOLVEMENT OF DIFFERENT AGENCIES
A multidisciplinary team: are a group of health and social care who
work together but have different roles and responsibilities. For example,
social workers, psychiatrists, doctors, and youth workers are examples of
the multidisciplinary team. This professional has specific service they
provide for their patient, and they treat various issues a patient may have
focusing on the issues in which they are specialised.
Multi-agency: they have an effective way in which they support families
and children with additional needs and support to help them secure
improvement in their life outcomes. This involves multi-agency panel
meeting in a regular basis to discuss those who require extra needs who
would benefit from the multi-agency.
There are many different agencies and professionals who might work
together to meet the needs of their service users, by providing them with
care using a different service provider which are able to deliver service for
individuals with various needs. An example would be Mr. R who is
suffering from stroke and epilepsy which is a physical need, and two
different professionals may be provided, an example is a care worker, and
consultant (stroke medicine) might work together to discuss and maintain

, and provide her with the right need and support. These two professionals
might work together to discuss and maintain the needs of the individuals
to maintain their needs and support.
Different agencies and professionals need to work together in order that
the needs of the service users are met as well as to provide a quick
response to their service users with multi and complex needs. Each
agency can be linked to one of the health and social care. There are many
organizations that is responsible for commissioning health and social care
within UK.
Meeting the needs of individuals requires the involvement of different
agencies because it might help maintain confidentiality between the
individuals and the agencies. In health and social care, the helps the
individuals to meet their needs. It is important that the agencies need to
work together to help individuals have and healthy and wellbeing lifestyle
that will help promote a longer life expectancy and a better quality of life.
They may be commissioning that are there to ensure that individuals have
the right resources and facilities to suit their needs and to help their
health in funding health service like the NHS. This commissioning within
the health and social care can help with Mr. R who is suffering from
epilepsy and due to this he likely to have unexcepted attack from his
illness which they may need an emergency like a paramedic. This
commissioning agency in the health and social care could Mr. R because
they will ensure he get all the requirement he needs. also, the Clinical
commissioning group within the health and social care help the service
user get better and have great needs within the care area. And this could
be a good link to Mer. James in my case study as he might needs to and
see his GP for his regular check up on his renal failure and which the GP
way offer help or do some test to ensure he is doing well.
Care Act 2014
The care act 2014 give fair care and support for individuals who have
physical, emotional, and mental well-being to their servicer provider and
the user who might need the available care. This could benefit Mr. R as it
could help MR. R family and friends to help in making decision his care
plans and needs that will support him while they may care for him.
