Respiratory system infection in elementary school kids (18 points)
John, - a ten year old comes home just before thanksgiving break from school feeling tired and
complaining of his throat hurting. There have been several cases of respiratory infection going around his
elementary school and his mom suspects that John may have gotten one too. At night, John’s condition

worsens- he has a fever of about 101 F and complains of his throat hurting upon swallowing. With
holiday travel plans ahead John’s mom decides to take him to the ER the next morning. By this time, he is
complaining of a headache and is not able to eat any solid foods.

Upon arrival at the healthcare facility, the nurse records John’s vital signs and asks his mom to describe
his health history. By this time, John’s face is red, and he describes his throat pain on a scale of 1-10 as an
8. The nurse first examines his oral cavity and the lymph nodes around the neck. The nodes are swollen
and painful during examination.

1. Do you expect this patient to have swollen lymph nodes? Explain why (1 point)

I would expect this patient to have swollen lymph nodes because of him complaining of pain
when he swallows.

2. What types of adaptive immune cells would be found in lymph nodes? (1 point)

B cells and T cells are adaptive immune cells that can be found in lymph nodes.

, John also has bad breath (although he brushed his teeth before coming to the clinic). His oral cavity is
red and swollen, and he has pus pockets on his tonsils. John experiences pain upon swallowing.

3. Do you think his throat is inflamed? Explain why (1 point)

I believe his throat is inflamed because it is swollen, red, and is generating pain.

Rapid Strep test (RST) is used to diagnose pharyngitis infection caused by “Group A Streptococci” (GAS).
The kits contain strips with antibiotics specifically against the proteins found on GAS. A positive result
will show two bands on the strip, and a negative result will show only one band. However, this test is not
always conclusive.

The nurse takes two swabs cultures of throat- with first she performs rapid Strep test, which shows two
bands.

4. Is the result positive or negative? Explain your rationale (1 point)

The result would be positive because two bands appeared on the test, indicating that it is
positive.

Next the nurse sends the second swab to clinical laboratory. She informs John’s mom that the test will
take two days, but John will need to start on antibiotics right away.
