In health and social care there is a strategy called personalisation, which helps
vulnerable adults to minimise their challenges and ensure they receive correct care
and support, set personal goals and have choices and control over their care and
support. Depending on who it is for there are different personalisations depending on
the person e.g. learning based or behavioural based. Personalised care plans can
also be used to identify the challenges faced by each person, and therefore develop
use of strategies to help overcome them. The process of personalisation involves
specific assessment of each individual and their needs in order to find out what
challenges they are facing.

Personalisation

As part of a healthcare setting, it means being able to resolve various challenges
that your service user can face each day by being capable of meeting their
expectations and thereby overcome those challenges with the correct approach.
However, it may often be difficult to recognise obstacles because you do not find the
best solutions to solve them, which is why you should encourage personalization as
a professional when interacting with your service user, which is the concept used to
ensure that each service user receives the care and support required to accomplish
their own personal goals by getting ownership of their own. The personalization is
that it works for the best interest of service users by encouraging them to monitor
their own treatment and also by helping professionals quickly recognise challenges.

In Valerie’s case, her challenges is the problem that makes her lifestyle difficult to
handle, one of these challenges involves her lack of motivation and low self-esteem,
so it will build her confidence by encouraging personalization with Valerie and
encourage her to feel more empowered to take a step forward and change her
lifestyle by being able to make decisions with the correct outcome. The
personalization process requires that if the service user has financial difficulties such
as in the case of Valerie, you should refer them to a specialist as their professional
who will help her with the local authority's national eligibility criteria assessment. In
order to decide whether the needs of the service user are available for assistance
based on whether there is a minimum standard that is the amount of conditions that
make them eligible for financial support. As in Valerie’s case, because of her mental
health and obesity, she completely met the standard of financial assistance as her
review covered her health, education, as she is a university student. Also, Valerie will
be involved throughout the whole process if necessary, with the help of an advocate.

In Billy’s case, his challenges is the problem that makes him lifestyle difficult to
handle, one of these challenges involved is that he is homeless and is visual
impairment, so it will build his confidence and make him live independently by
encouraging personalisation with Billy and encourage him to feel more confidence
about himself and empowered to take a step forward and change his life. The
personalization process requires that if the service user has financial difficulties such
as in Billy’s care, the social worker should refer him to a specialist as their

professional who will help her with the local authority's national eligibility criteria
assessment. There are some conditions that make people eligible for financial
support. As in Billy’s case, because he is visually impairment, he meets the standard
of financial and physical assistance. Given that he is eligible, the local authority will
start to care, support, plan and discuss with Billy how he wishes to live his life. Also,
the local authority will look at preventative services, such as information about
support available, such as help in the home given that Billy is visually impaired will
need some kind of help at home when the social worker will find Billy a house to
stay.

In Aisha’s case, her challenges is the problem that makes her lifestyle difficult to
handle, one of the challenges involve is that she suffer from dementia and she lives
alone at home, so she will live a healthy lifestyle and her mental state will also
contribute by encouraging personalisation with Aisha and having her in a care home
would ensure that she is safe. The personalization process requires that if the
service user is physically or mental impairment such as in Aisha’s case, a doctor
would contact a care home and refer Aisha’s case then an admiral nurse will work
with Aisha to help her with the local authority’s national eligibility criteria assessment.
There are different conditions to see if a person is eligible to have care and support.
As in Aisha’s case, because she has dementia which has gotten more severe from
when she has been admitted to the hospital and she lives alone which means she
doesn’t have no one that can take care of her, and this makes her eligible to have
care and support given that she has met the standard.

Plans

In order to reduce the challenge the service user is facing and promoting
personalisation you could come out with a strategy that involves plans that recognise
service user preference.

A learning plan states that the plan is the customization and adaptation of
educational methods and techniques so that the learning process is better suited for
each individual learner, with their own unique learning style, background, needs, and
previous experiences. In Valerie’s case, the learning plan will be the right solution
since this is an educational approach that aims to customize learning for every
person's strengths, needs, skills, and interests. Valerie would get a learning plan
based on what she knows and how she learns best. The social worker needs to
understand exactly what Valerie’s short and long-term goals are, cover her strengths,
weaknesses, skills, and knowledge gaps. After this, the social worker can set out the
learning plan that is best for Valerie. It is the roadmap that Valerie will follow to reach
her educational or training goals. The first step is to assess. A personalized learning
plan always begins with an assessment. Assessments can be made for specific roles
or at a team or departmental level and can be used for several employees at the
same time. The social worker should assess what information should be known, at
what level, and should be able to identify specific knowledge gaps. Once Valerie has
completed her assessment, this data can be input into the system, and the
development of a learning plan can begin. The second step is to determine the goals
and skills needed for each specific role. Now that the social worker knows where
