UNIT 5MEETINGINDIVIDUAL CAREAND SUPPORTNEEDSASK 1 SECTION A INTRODUCTIONTherearemanydifferentwaystosupportindividualsinthehealthandsocialcaresettingsandhowtomeettheirneedsprofessionally.Someofthekeypointstomeetthismissionistoapplycarevalues,promoteequalityanddiversityintheworkplace,followrulesandregulationsandtoensurethattheirownchoicesandpreferencesarebeingadheredwhenplanningfortheircareandtreatment.Inthisassignment,Iwillbediscussingaboutwhatarethepossiblewaystomeetindividuals’needsandhowarewe(ashealthcareprofessionals)goingtoensurethatequalityanddiversityarebeingappliedduringtheprocess.Tobetterunderstandthetopics,therearevariouscasescenariosincludedfor each key points.Case Scenario 1NusratPatelis19yearsofageandhaslearningdisabilitiesandepilepsy.Shehasrecentlyleftherresidentialschoolandnowlivesfulltimewithhermother.Nusrat’smotherhasrecentlygivenupworktocareforher,butisfindingthisverystressful.Nusrat’sfatherdiedwhenshewasfiveyearsold.Nusratnowattendsthecommunity centre.StaffandvolunteersatthecommunitycentreareawareofNusrat’srighttobeinvolvedindecisionsaboutherowncare.However,asshehaslearningdisabilities,itwasdecidedtoalsoinvolveNusrat’smother,ashermaincarer.Nusratsometimeshasdifficultyinexpressingherownneeds,wantsandwishesandtheteamhasincludedanadvocate who has experience in working with individuals who have learning disabilities.Nusrathasherownpersonalcareplanatthecentre,whichhasbeenformulatedtakingaperson-centredapproach.TheplanfocusesonNusrat’sabilitiesratherthanherdisabilitiesandisflexibletoincorporateanynecessary changes.TheplanincludesthedesignationofanamedpersonNusratcancometoifshehasconcerns.Nusratbecomesfrequentlydistressedandsothenamedpersonhascompletedacounsellingtrainingcourse,whichincorporatedpositive communication skills, in order to support her.TheplanningprocessforNusratincludedgraphicalfacilitationsothatacircleofsupport,whichincludesNusrat, her mother and the named person, could be designed. Other individuals may be added as appropriate.Nusratenjoysdanceanddramaandhasjoinedactivitiesatthecentre.TheteamwhorunstheactivitieshasproducedMakatonsheetsforanyscriptsorroutinestobefollowed,sothatNusratcanfullyparticipateinactivities and productions.Nusrat’sepilepsyisnot,currently,wellcontrolledandithasbeenagreedthatavolunteerwillaccompanyNusrattothebathroomtoenableemergencyactionifthisisneeded.Thevolunteerisawarethatshemustbediscreet in order not to compromise Nusrat’s right to dignity.WiththeagreementofNusratandhermother,thecentremanagerhasarrangedameetingwithNusrat’sG.P.todiscussimprovedmanagementofNusrat’sepilepsy.TheG.P.hasnowreferredNusrattoaspecialistnursepractitioner, who is an expert in epilepsy.Nusrathasatrustingnatureandislikelytoattachherselftoanyonewhoappearsfriendly.Thishasraisedsafeguardingconcernsandsoalistof‘friends’usingphotographsandMakatonsymbolshasbeenprovidedforNusrat, to support her independence, whilst contributing to her safety
