Aetiology.

ADHD is a complex interaction of neurobiology and environmental factors.
It often runs in families and has significant research suggesting genetic
dispositions with a mean heritability rate of 76% (Johnson et al,2020 in
NICE,2025). SH has mentioned that her parents had also presented similar
symptoms in childhood. ADHD is said to change with age, symptoms are
often fluctuating but still cause significant impairment in people’s day-to-
day life.

Given that SH is perimenopausal, it is important to consider the biological
impact on her health. Menopause is caused by a decrease in the
production of progesterone and oestrogen (NICE,2018). These hormones
play a significant role in the maintenance of the emotional health of
women, often increasing the prominence of ADHD symptoms in women
(Osianlis et al., 2025) earliest known references for menopause are
scarce, with it being described in the 1930’s as a deficiency disease.
Menopause tends to occur later in Western nations than in developing
regions such as India, where the average age falls between 45 and
47 years. In earlier periods, women generally experienced menopause at a
younger age, but contemporary figures show the typical range has shifted
upward to roughly 50–51 years. Attitudes also differ markedly: women in
Western societies often view menopause in a negative light, whereas in
countries like India it is commonly regarded more positively (Singh
