This simulation was very interesting because you were able to see the patient after they received their treatment so you were able to notice the difference between the first encounter and now the discharge encounter. 3. Talk about what went well in the scenario. The things that went well in the scenario was that I was able to answer any concerns that David or his mother could have regarding the discharge plan by educating them about his new medication regimen and different symptoms and signs to be aware of while taking the medication. 4. Reflecting on David Carter’s case, were there any actions you would do differently? If so, what were these actions and why I think that throughout the conversation I repeated a lot of the questions relating to the Mental Health Exam which could’ve started to aggravate David. I also feel like I should’ve further educated the mother about signs to look out for regarding David’s diagnoses because he had mentioned that he was still unsure on if he was going to eat the food at his house due to the poison. I would educate that it sometimes takes time for the medication to have an effect that he has made a great improve over the last 2 weeks. Scenario Analysis Questions: 1. What needs to be stressed during discharge teaching related to his new medication regimen (sertraline and fluphenazine)? Side effects such as increased risks of suicidal thoughts for sertraline, as well as the signs and symptoms of Serotonin Syndrome. For fluphenazine, it is important to stress that they medication has to be taken at night because of the side effects of drowsiness. 2. Identify community resources in your area that are available to David Carter and his family. One should be for David Carter, one for his mother, and one for the family. Mr. Carter can join a peer self-help group such as Schizophrenia Alliance so that he is able to share his experience and gain support with other people with the same illness. His mother and family can participate in support groups in order to better understand Mr. Carter’s illness and condition. 3. What other discharge teaching is recommended for David Carter and his family? Other than teaching about the new medications and their side effects, it is recommended to discuss follow-up appointments to ensure compliance and potential problems can be addressed. Concluding Questions: 1. How would you apply the skills and knowledge gained in David Carter’s case to an actual patient situation in different acute care units (emergency room, intensive unit, obstetrics unit, etc)? I would apply the skills and knowledge gained in David Carter’s case to an actual patient on different units by making sure to use therapeutic communication and try to stay within the boundaries of reorientating the patient about false situations.
