Risk: Low (phq-9 item 9=0; no
plans/preparations; no current or past
self-harm or suicidal intent)
, PSS:
My main problem is (what): Feeling low. Hard
to do things I enjoyed or manage around the
house.
This happens when (trigger): My knee still
hurts from surgery. I don’t get out much.
Physically I feel (A): Tired, slow.
Emotionally I feel (E): Sad, lonely.
I have thoughts that (C): I’m losing my
independence. I might end up on my own.
In order to cope I (B): Rest a lot. Leave things
for another day.
I avoid (B): Going out. Talking to people. Doing
C O N C E P T UA L I Z AT I O N things around the house.
As a result, this has impacted: I don’t do much
now. I feel more tired and alone.
Goals
1. Spend at least 1 hour twice a week playing
with my granddaughter
2. Get back to doing little things around the
house, like cooking or pottering in the garden.
3. Speak to people more, calling my cousin once
a week or see someone now and then.
