How to Use This Test Bank
Work through the questions in Part 1 without notes, then check yourself against the answer key in Part 2 and
read the full rationale in Part 3. Reading the explanation for every option you got wrong (and the ones you
guessed right) is where most of the learning happens.
Difficulty tags
Application - apply a fact or calculation to a realistic scenario.
Analysis - interpret findings, distinguish look-alike concepts, or spot the flaw in a plausible option.
Prioritization - decide what to do first or what must not be missed (the NCLEX-style "best action").
Suggested study plan
• Take one chapter block at a time (4 to 8 questions) and score it on the tracker in Appendix B.
• Re-do any chapter below 80% after reviewing the cited pages.
• Use the High-Yield Numbers sheet (Appendix A) for last-minute review before skills check-offs and
exams.
• Calculation items (Chapter 5): set up dimensional analysis on paper, then confirm against the
rationale.

Part 1 | Questions
Chapter 1: General Survey
1. [Prioritization]
A nurse walks into a patient's room and finds the patient has just lost consciousness. Which action is the
priority in the vital sign sequence?
A. Obtain a tympanic temperature
B. Apply a blood pressure cuff and obtain a manual reading
C. Place the pulse oximeter probe and wait for a stable waveform
D. Check the carotid pulse
2. [Analysis]
An RN delegates routine vital signs on a stable patient to an unlicensed assistive personnel (UAP). Which
statement best reflects the RN's responsibility?
A. The RN remains accountable for analyzing the results and instituting appropriate interventions
B. Accountability transfers to the UAP once the UAP has been trained
C. The UAP may decide whether an abnormal value needs to be reported to the provider
D. Vital signs may never be delegated in any practice setting
3. [Application]
A patient finished drinking a cup of hot tea 5 minutes ago. The nurse needs an oral temperature. What is
the best action?
A. Measure now, because the 5-minute interval applies to hot beverages
B. Delay the oral measurement until 15 to 25 minutes after the beverage (or use another route
according to agency policy)
C. Measure now and subtract 0.5°C from the result
D. Obtain a rectal temperature, since it is required whenever oral readings are unreliable in adults
4. [Application]
A nurse obtains a tympanic temperature of 37.6°C. Based on the usual relationship between tympanic
and oral readings, the patient's oral temperature would be expected to be approximately:
A. 37.9 to 38.2°C
B. 36.0 to 36.4°C
C. 37.0 to 37.3°C
D. 37.6°C (the two routes are identical)
5. [Prioritization]
A patient with COPD is receiving oxygen at 2 L/min by nasal cannula. SpO2 is 90%, respiratory rate is 16,
and the patient is resting comfortably. What is the nurse's best action?
A. Increase the flow to 6 L/min until SpO2 reaches 100%
B. Switch to a non-rebreather mask at 15 L/min
C. Activate the rapid response team for hypoxemia
D. Continue the current oxygen therapy and monitor, because 88% to 92% is the usual target range for
COPD
