Week 2: Reading/PPT notes
Chapter 6: Objectives
● Discuss how the nursing process is used in the administration of meds
○ Example: WHen a nurse is evaluating whether the pain medication given to the patient has had therapeutic
effects and relieved the pain, the nurse relies on assessment skills to evaluate whether the patient is pain
-free or has obtained some relief from the drug
● Nursing process
○ Assessing
○ Diagnosing
○ Planning
○ Implementing
○ Evaluating
● Assessment of the patient
○ Systematic collection, organization, validation, and documentation of pt data
○ Health history and physical assessment
○ Baseline data gathered; will be compared to later information from observations
■ subjective/ objective data
○ Health history tailored to patient’s clinical condition
■ Includes use of all meds/supplements taken
○ Physical assessment
○ Once pharmacotherapy is initiated, assessment focuses on reaction to med. Watch for:
■ Desired response
■ Adverse effects
■ Pt capability of assuming responsibility for self-admin
○ Herbal and vitamin supplements must be monitored
■ Can increase or cancel effect of prescribed meds
● Nursing diagnosis for drug admin
○ Often most challenging part of nursing
○ Focus is on pt’s needs, not nurse’s
○ Three main areas of concern in relation to pharmacotherapy
■ Promoting therapeutic drug effects
■ Minimizing adverse drug effects and toxicity
■ Maximizing patient ability for self-care (including knowledge, skills, and resources necessary for safe
and effective drug admin)
○ May focus on pt’s response to actual or potential health and life processes
○ Risk diagnosis includes:
■ Diagnostic statement
■ Related factor or inferred cause
○ Actual diagnosis includes:
■ Diagnostic statement
■ Related factor or inferred cause
■ Evidence to support statement
● Most common nursing diagnoses for med admin
○ Deficient knowledge
, Week 2 Pharmacology
■ Pt not properly educated about med
○ Noncompliance***
■ Pt properly educated, but chooses not to take med
● Some diagnoses for drug administration
○ Activity intolerance
○ Pan, acute, or chronic
○ Risk for falls
○ Urinary incontinence
● Nurse Planning phase:
○ Prioritizes diagnoses, formulates desired outcomes, and selects nursing interventions that can assist patient
to establish an optimum level of wellness
○ Goals for patient receiving meds
■ Based on nursing diagnosis
■ Focused on what pt will achieve or do
■ Prioritized
■ Discussed with patient or caregiver
■ Can be short- or long-term goals
■ Goals focused on
● Safe and effective admin
● Therapeutic outcome
● Treatments of side effects
■ Outcomes for patients receiving meds
● Provide specific, measurable criteria
● Evaluate degree to which goal is met
● Focus on what pt will achieve or do
● Are discussed with patient or caregiver
● Should be written and should include
○ Subject (pt)
○ Action required by subject
○ Circumstances
○ Expected performance
○ Specific time frame
● Implementation phase actions
○ Nurse applies knowledge, skills, and principles of nursing care to move pt toward desired goal and optimal
wellness
○ Aim of interventions:
■ Pt returned to optimal level of wellness
■ Safe and effective administration of meds
○ Administer med
○ Continue assessment of patient
○ Monitor drug effects
○ Carry out interventions in planning phase
○ Provider patient teaching
○ Monitoring drug effects is primary intervention
■ Monitor for identified therapeutic effect
■ Reassessing patient
● Physical condition
