Discharge Plan Checklist
I. Current Mental and Physical Health Status:
Briefly describe the client’s current mental and physical health, including diagnoses,
treatment regimens, and current functioning.
Amella Abraham is a 16-year-old transgender African American girl diagnosed with
moderate Other (or Unspecified) Stimulant Use Disorder, Nicotine, and Other Specified Gender
Dysphoria. Upon admission, she reported a constant craving and compulsive use of nicotine,
where she consumed the substance more than 12 times a day. She also noted how she could not
avoid these behaviors because it was her coping mechanism to overcome stress. Since
childhood, Amella identified as a girl despite facing discrimination, rejection, and invalidation
from her family members and her peers. She also reported emotional pain and stress following
how she was treated because of her gender identity. Upon assessment by a multidisciplinary
team, Amella was referred to a treatment session, an outpatient treatment for substance use and
mental health. She also attended counselling sessions with her parents. Amella participated in
the sessions for a year, and after completion, she reported significant improvements, including
discontinuing nicotine use. Although she reports taking marijuana occasionally, she seems to
have improved coping strategies. Amella is stable and seems motivated for her future career
progress.
