PURPOSE OF MEDICATION
Expected Pharmacological Action Therapeutic Use
Directly converts plasminogen to plasma Lysis of thrombi in coronary arteries, with
which then degrades clot bound fibrin. improvement of ventricular function and
reduced risk of heart falure or death.
Complications Medication Administration
CNS: intracranial hemorrhage EENT: epistaxis, gingival IV 15mg bolus then .75mg over
bleeding Resp: bronchospasm, hemoptysis. CV: 30 minutes, then .5 mg over
reperfusion arrhythmias, hypotension, recurrent next 60 minute.
ischemia/thrombolism Stroke: IV .9 mg given as
infusion over 1hr with 10%
given as bolus over 1st minute
Contraindications/Precautions
Active internal bleeding, history of cerebrovascular
accident, intracranial or intraspinal injury,
Nursing Interventions
Begin therapy asap after onset
of symptoms.
monitor VS including temp,
continuously for myocardial

Drug: aspirin other NSAIDs, warfarin, heparin, and he[arin

Explain purpose of medication
and the need for close
Evaluation of Medication Effectiveness monitoring to patient and family.
Lysis of thrombi and restoration of blood flow Instruct patient to report
prebention of neurologic sequelae in acute ischemic stroke bleeding or bruising
cannula or catheter patency Explain need for bedrest and
minimal handling during therapy
to avoid injury.
