Module 03 Assignment - Interventions to Promote an Environment of Safety Mr. Joe Smith, 75-year old man, recently suffered a left cerebrovascular accident. He has right visual field deficits and residual weakness in this right arm and leg. He uses a walker for mobility. He takes Metoprolol for his blood pressure and the physician started him on Coumadin because he has recently been diagnosed with atrial fibrillation. Diagnosis: Activity intolerance related to decreased mobility as evidenced by utilizing a motorized wheelchair. Expected outcomes: Patient will utilize manual wheelchair 2 times a week to help promote arm exercise and independence. Interventions: 1. Patient will participate in PT/OT per orders. 2. Patient will demonstrate proper use of manual wheelchair 3. Patient will demonstrate range of motion exercises and perform ROM 4. Patient will show an increase in independence with ADL’s. 5. Patient will ask for assistance with ambulating in the halls Goals: Still in progress, continue to monitor patient and promote independence. Patient will show increased strength within two weeks of PT/OT Diagnosis: Disturbed sensory perception Expected outcome: Patient achieves optimal functioning within limits of visual impairment Interventions: 1. Ask patient about specifics such as ability to read and see television 2. Assess central vision with each eye, individually and together. Vision loss may be unilateral, bilateral, and may not affect both eyes.
