Patient: 22-month-old child

Reason for Encounter: Difficulty making friends / social interaction concerns
History
Chief Concern:
Parent reports the child has difficulty interacting with other children and does not seem interested
in playing with peers.
History of Present Illness:
The parent states that the child prefers to play alone and does not initiate interaction with other
toddlers during playtime at daycare or family gatherings. The child occasionally plays alongside
other children but does not share toys or engage in cooperative play. Parent reports minimal eye
contact with unfamiliar children but good interaction with caregivers at home. No regression of
skills noted. Symptoms have been present for several months.
Developmental History:
• Gross motor: Walks independently, climbs furniture
• Fine motor: Able to stack blocks, feeds self with fingers
• Language: Says 10–20 words, follows simple commands
