Identifying information Name: Kyle Jones Sex: Male Ethnicity: Italian American Sexual Orientation: Heterosexual Age: 45 years old Handedness: Right-Handed Occupation: College English Instructor Grade Level: Doctorate PhD II. Reason for referral Mr. Jones was recently involved in an automobile accident where his car was struck from the rear end while at a stop light. Mr. Jones wife has also shown some concern due to unfamiliar social behavior that has occurs since her husband’s accident. III. Current Symptoms/Presenting Concerns Although he claims that he remained conscious during the incident Mr. Jones has shown signs of memory loss, for example he does not recall any events that occurred immediately following the accident. Mr. Jones is said to be a very socially active person according to his wife, but she has noticed that he has been much more withdrawn in social environments than usual since the accident and was not making logical decisions. A week after the accident Mr. Jones cognitive issues have somewhat improved although he still deals with reduced attention span, disorganization, losing track of tasks and thoughts, and reduced ability to generate new ideas (Kennedy&Harper,2014) which may affect him financially as well because he is unable to put together or write lectures for his class. There has been occasional episode of memory loss as well as hand and feet coordination issues. Mr. Jones also stated that he has dealt with a few incidents in the past that involved bodily harm. As a teen he was involved in an automobile accident which occurred while driving with friends and resulted in one of his friends being to be thrown through the windshield and dying due to not wearing a proper restraint. He also admits to being dropped on his head as a child which caused a moment of unconsciousness. IV. Psychosocial History Kyle seems to be a very well-educated man and has never been diagnosed with any learning disabilities. Which is not surprising seeing that he earned a Ph. D in English and has been a college professor for the past 30 years. Even with his recent health issues he continues to work as well as participating in the institutional review board which has become a task since the accident. Jones doesn't come from a long line of neurological disorders besides his maternal aunt. However, many of his immediate family have died due to other illnesses such as cancer. His mother died of cancer at 41 and his brother died last year from colon cancer, also his two sisters were recently diagnosed with thyroid cancer. As of now neither of his two children have shown any signs neurological issues or any other illness at this time. Mr. Jones stated that the has never have any issues with substance abuse but does enjoy a glass of wine. Mr. Jones also admits to having a history of depression that may have begun in childhood but insist that he never thought of self-harm. These depression episodes last for about 3-6 months the most recent was when he came into contact with the friends from high school who were also involved in the accident that killed their friend. It seems that Mr. Jones may be suffering form either post-traumatic stress or post traumatic amnesia. Traumatic brain injuries can have a devastating impact ranging from impaired physical capability to altered cognition, which impedes functional independence. Post-traumatic amnesia is characterized by a state of disorientation and confusion of cognitive functioning following traumatic brain injury which include restlessness, inability to recall events, poor attention, and agitated behavior (Nhung&Harper,2019). Mr. Jones has displayed many of these signs during his lifetime. Post-traumatic stress may also come from various stressful situations he has endured which may be the reason why it has become so severe.
