, Chief Complaint:
“Sore throat and fever for 2 days.”
HPI (History of Present Illness)
Amanda Wheaton is a 23-year-old female college student who presents to the
outpatient clinic with a 2-day history of fever, sore throat, and painful
swallowing. She reports that the sore throat began gradually and worsened
yesterday. She describes the pain as constant, burning, and rated 6/10, worse
with swallowing solids. She reports associated fatigue and “swollen glands.”
She denies cough, rhinorrhea, nasal congestion, ear pain, shortness of breath,
chest pain, abdominal pain, or rash. She denies sick contacts except for “several
girls in my dorm who have been sick with colds.” She has taken ibuprofen with
mild relief of fever.
No recent travel. No antibiotic use in past 3 months.
PMH (Past Medical History)
• Seasonal allergies
• No chronic illnesses
• No history of recurrent strep infections
PSH (Past Surgical History)
• None

• Ibuprofen 400 mg PRN
