PAEDIATRICS
The role of doctor in paediatrics
- Always assess the child’s psychosocial : How things at school ,Child’s self-esteem, who
else taking care of home
- Safeguarding
APPROACH TO PAEDIATRIC HISTORY:
- GRIPS, (in station you will not see the child, you will talk to the mother, nanny..)
Always validate the mother’s concern
- ODPARA
- DD: if you cannot think of any DD, think of systemic paediatric review DPAMGUU:
o Dehydration : less active, lethargic, when was the last time you change his
nappies
o Pneumonia
o A: AOM Acute otitis media : cries while pulling his ears, pain in ears
o Meningitis: fever rash photophobia high pitch inconsolable crying
o G: gastroenteritis: tummy upset or pain, how are the stool like, vomiting
o U: upper respiratory infection
o UTI: cries when he goes to pee, any smell or change in colour
- Paediatric history: PBIND
o Pregnancy: even when speaking to father
o Birth: how was the birth? Did the child require any assistance/ special delivery/
was he admitted into any special care setting/ when? How many weeks pregnant
were you when you delivered your child
o Immunization: Is the child up to date with his jabs
o Nutrition: is the child eating well
o Development: Are you happy with the way your child is developing?
- MAFTOSA: MAM (medical, allergy, medication), family history
- Assess the child psychosocial: who lives at home? Do you bring your child to baby
toddler groups (1 to 2 yeaold)? Nursery (3 and above)? How is the child at school? Ask
if the teacher has raised any concern at school? Ask about siblings: How are they?
- Assess the child’s self esteem in some stations (e.g skin station)
