An attempt to reduce job-caused stress is being made to lower the costs 
associated with this increasingly worrying issue for companies. An estimated 13.5 billion working days were lost in 2001 due to work related stress, anxiety and depression. The CBI Survey is keeping a close look at the figures related to this issue, measuring the costs and negative effects of stress. The Health and Safety Commission has also made attempts to lower these effects.

In 1999, the Institute of Medicine reported that 44,000 to 98,000 patient deaths were the result of preventable medical errors. This makes people try to find who is at fault in order to correct the error, but the Institute of Medicine had a somewhat different approach, their proposed solution involved both system errors and individual error in order to take corrective action. They concluded that, in order to correct this issue, a change in culture was required.
