supervision or consultation in planning your group facilitation. Did you consult the
textbook, discuss ideas with your group or a friend, ask the professor, etc.?

Group Goals and Session Goals
Type out the goals for your overall support group. Explain how your topic relates to the
overarching goals of the group. What did you hope to accomplish with this session?

Icebreaker
What icebreaker did you use? How did it relate to the topic? Was it effective
in engaging every participant?

Relevant Content and Material
Explain briefly any material that you presented. Did it succeed in providing meaningful
material to prompt group discussion?

Activities/Exercises
Explain the exercises you utilized. For each activity, explain:
a) Why did you select those activities?
b) Did they help you accomplish your goal? Share observations of the group to
support your assertions. Please note that typically some exercises work really
well and help move the group closer to meeting one or more of its goals.
Other exercises may not have gone as well as you intended.
c) Which activities were most valuable? Which ones might be omitted or
modified to better meet your goal?
