Week 6: APN Capstone Portfolio Part 2 Guidelines & Rubric PURPOSE Purpose The purpose of the APN Capstone Portfolio is to demonstrate your professional growth and expertise by providing evidence of the knowledge you have learned. The Professional Portfolio serves the following purposes: Promotes the synthesis of knowledge – To become a competent leader in the nursing profession and a selected specialization, you need to synthesize a great deal of information from a variety of sources. Promotes reflective thought – One hallmark of graduate education is the ability to think analytically and engage in self-assessment. Creating the portfolio allows you to evaluate and demonstrate your achievements in acquiring advanced nursing and specialty knowledge. Provides evidence that assignment, course, and program learning outcomes have been met –The portfolio reflects the different ways you have met the learning outcomes. Part 2 Directions – This assignment should be an entire document in either MS Word or PDF format with a title page noting the assignment name, student name, course, etc. The assignment should also include a table of contents denoting what of the below documents are on which pages. APA format and writing at the graduate-level is expected. Part 2.1: Synthesis of Knowledge (100 points) 1. Using work that you have completed during the MSN program here at Chamberlain University, gather your prior submissions and identify a minimum of 10 exemplars of your graduate work. 2. Use the worksheet below to copy your exemplar and to identify with Chamberlain Program Outcome, MSN Essential, and NONPF Competency that each exemplar meets. 3. You must include at least 10 exemplars, and as a group, each Chamberlain Program Outcome, MSN Essential, and NONPF Competency has to be addressed at least once. 4. If you do not have an exemplar that meets these criteria, you can write an exemplar in the area that is not represented by your prior work. 5. Identify at least 2 concepts from each exemplar and how they connect to the Chamberlain Program Outcome, MSN Essential, and NONPF Competency that you have identified. 6. Reflect on how you have met the outcome, essential, and/or competency for each exemplar and how it has transformed you as a prospective masters-prepared advanced practice nurse. 7. Combine the 2 concepts from each exemplar and create a concept map on how these concepts interconnect and interrelate to each other in the framework of the NONPF Competencies. .......................................CONTINUED.................................
