the form prescribed by Department of Justice, and shall be available to respond to

D) Failure to report is guilty of misdemeanor punishable by up to 6 months
confinement in county jail or by a fine of $1,000 or by both that imprisonment and
fine
E) Failure to report that is resulting in death or great bodily injury shall be punished
by not more than one year in county jail, by a fine of not more than $5,000, or by
both that fine and imprisonment.
Objectively reasonable for a person to entertain a suspicion, based upon facts that
