Semester 2.2, theme 10
WEEK 37: Swelling of the groin and testes
Understand the anatomy of the inguinal canal, the external and
internal genitalia MANUAL
Insight into the embryology of the inguinal canal, the external and
internal genitalia
1. Cloacal membrane initiates signaling for genital development
2. Thickening of mesothelium medially to mesonephros = gonadal ridge
3. Gonadal cord grows into mesenchyme
4. Primordial stem cells enter the cord in week 6
Week 7: development of genitalia starts.
- In males: gonadal cord becomes seminiferous cords
- Female sex development is NOT dependent on hormones, males’ is
- AMH: suppresses development of paramesonephric duct
- Fusion of müllerian duct = broad ligament
Remnants:
- males: mesonephric duct: appendix of epididymis and paradidymis
Paramesonaprhric duct: appendix of testis and prostatic utricle
- female: mesonephric duct: appendix vesiculosa
paramesonephric: vesicular appendages, hyditia
Urethra is derived from endoderm, only the distal part in males is from
ectoderm.
Inguinal canal: forms pathway for testis to descend, develops in both sexes
Insight into the causes, diagnostics and treatment of scrotal swelling
, A lump in the scrotum is malignancy until proven otherwise!
Diagnosis: LDH measurement(see if it is metastatic), US, HCG (tumor), AFP (yolk sac tumor), PE, bloodtest
Insight into the acute pathology of the scrotum
Testicular torsion until proven otherwise!
