• Why are first-case surgeries being delayed?
One significant cause of first-case delays is inefficient scheduling of the surgeries.
• Why is the scheduling inefficient?
Some first-case start delays result from patients being transported to the labs by staff
instead of in the preoperative area. Patients must have all preoperative work done quickly
before they come to the lab by surgical team members. This is considered a poor utilization
of resources. Some poor communication results from nurses in the preop and labs area not
being able to let each other know when tasks are completed and when others still need to
be finished by providers.
• Why is scheduling between teams and coordination poor?
In some cases, vendors are needed for specific specialized equipment. In other cases, they
may require anesthesia. There have been times when both groups have not been made
aware of the case, and communication must be made to ensure the case is completed. In
the first case of delays that must be transported directly to the lab and not to the
preoperative area, doctors may not always communicate that to staff beforehand.
• Why are there no standardized processes for communication and scheduling?
One reason for the poor communication is a lack of training from all caregivers involved.
Some physicians report that there is no way to communicate their needs in the current
scheduling system.
• Why is there a lack of a standardized process for scheduling and communication?
The surgical scheduling software has existed for many years and could be outdated. There
could be a lack of process improvement from leadership in the OR areas.
