address Sarah's healthcare challenges. Hence, a collaborative team approach is necessary to
provide Sarah with comprehensive care that addresses her overall well-being. By sharing
information and coordinating care, the team can develop a unified treatment approach that
supports Sarah at home, in therapy, and at school (Slusser et al., 2019).
Efficient collaboration within Sarah's healthcare team is crucial in ensuring that her care is
consistent and devoid of gaps. The team has embraced a transparent approach to communication,
enabling every member to understand the roles and plans of their colleagues. This approach
facilitates smooth transitions for Sarah between providers and settings. For example, when her
speech therapist was running late, the occupational therapist was promptly informed and
extended their session to incorporate the goals of both therapies. This ensured a seamless
handover to speech therapy. Additionally, the team can quickly troubleshoot issues, such as
when Sarah's grandmother reported behavioral problems at home. The team came together to
