collaborative practice, commitment

Commitment - a state of being obligated or emotionally compelled. Organizational commitment
is the strength of an employee's relationship to, sense of belonging to an organization

Work ethic: - the belief and importance in the moral worth of work.

Components of work ethic include:

-High value on hard work and diligence

- Apply self to work tasks

-Try to get the task/job right the first time

-Demonstrates accountability and responsibility

Types of event - ANSWER Breach in care or breach of duty => related to healthcare, occurs when
one deviates from the standard of care.

"Near miss": are caught BEFORE they reach the patient and cause harm.

Adverse events: undesirable events associated with medical care. Adverse events have the
POTENTIAL to become sentinel events.

Sentinel events: often cause serious physical harm, disability, or death to the client as defined
by The Joint Commission.

RCA is a formal process to:

1) define errors

,2) identify risks

3) develop and test prevention strategies

4) implement and adopt those strategies.

Think of it as a problem solving technique that can be performed to investigate incidents to
identify breakdowns in processes and systems to prevent future occurrences.

RCAs are:

-Extremely detailed

-Provides an assessment of systems and processes

-Often carried out by an interdisciplinary team (facilitated by Risk Management)

-Preferably non-punitive

***Hospitals accredited by The Joint Commission are required to perform RCA on all sentinel
events***




General Steps for completing RCA

Step 1: Identify the event to be investigated and gather preliminary information.

Step 2: Select team facilitator and team members

Step 3: Describe what happened. Collect and organize the facts surrounding the event to
understand what happened.

Step 4: Identify the contributing factors

Step 5: Identify the root causes. A thorough analysis of contributing factors leads to
identification of the underlying process and system issues (root causes) of the event.

Step 6: Design and implement changes to eliminate the root causes. The team determines how
best to change processes and systems to reduce or prevent the likelihood of another similar or
harmful event.

Step 7: Measure the success of change(s). Like all improvement projects, the success of
improvement actions is evaluated. The RCA should reduce the risk of future harmful events by
minimizing or eliminating the root causes.
