Collaboration Café – The RN’s Role in Health Assessment: A Reflection
In my capacity as a Critical Care Nurse, I routinely conduct thorough health assessments on all my
patients within the intensive care unit (ICU) setting. These assessments encompass a wide array of
parameters including vital signs, neurological status, cardiovascular and respiratory function, among
others. Beyond the conventional head-to-toe examination, I employ focused assessments to delve
deeper into specific areas of concern, particularly when irregularities or early signs of deterioration
manifest. Such assessments are particularly pertinent in critical care scenarios where patient acuity
levels are high and timely intervention is paramount. The fast-paced nature of critical care demands
early and swift recognition, accurate and timely diagnosis, and immediate implementation of
appropriate interventions to avert adverse health outcomes for our patients. Therefore, conducting
comprehensive and focused health assessments is not only appropriate but indispensable in optimizing
patient care within critical care settings.

shift in the ICU. A patient under my care exhibited early manifestations of septic shock stemming from a
newly diagnosed urinary tract infection (UTI). Through astute observation and meticulous assessment, I
discerned crucial indicators suggestive of urosepsis such as hypotension, tachycardia, fever, and an
altered mentation. Swift communication with the healthcare provider facilitated prompt initiation of
life- saving interventions including fluid resuscitation, administration of broad-spectrum antibiotics, and
early vasopressor support. This encounter underscored the pivotal role nurses play in conducting
comprehensive assessments and promptly communicating pertinent findings to the healthcare team,
thus optimizing patient outcomes. The experience emphasized the imperative of health assessment
proficiency in facilitating timely interventions and mitigating adverse health sequelae for patients in
critical condition.
