Long-term care (LTC) - Answer-Given in long-term care facilities (LTCFs) for persons who need 24-hour skilled care. Skilled care - Answer-Medically necessary care even by a skilled nurse or therapist; it is available 24 hours a day. It is ordered by a doctor and involves a treatment plan. This type of care is given to people who need a high level of care for ongoing conditions. Length of stay - Answer-The number of days a person stays in a care facility. May be short, such as a few days or months, or longer than six months. Terminal illness - Answer-The illness will eventually cause death. Chronic - Answer-The conditions last a long period of time, even a lifetime. Most people that live in longterm care facilities have this kind of condition. Diagnoses - Answer-Medical conditions determined by a doctor. Home health care - Answer-Health care provided in a person's home. Generally given to people who are older and are chronically ill but who are able to and wish to remain at home. Assisted living - Answer-Facilities where residents who need some help with daily care, such as showers, meals, and dressing. People who live in these facilities do not need 24-hour skilled care. Adult day services - Answer-For people who need some help and supervision during certain hours, but who do not live in the facility where care is provided. Acute care - Answer-A 24-hour skilled care given in hospitals and ambulatory surgical centers. It is for people who require short-term, immediate care for illnesses or injuries.Subacute care - Answer-Care given in hospitals or in a long-term care facility. Used for people who need less care than acute illness, but more care than for chronic illness. Outpatient care - Answer-Usually given for less than 24-hours for people who have had treatments or surgery and need short-term skilled care. Rehabilitation - Answer-Care given by specialists. Physical, occupational, and speech therapists restore or improve function after an illness or injury, Hospice care - Answer-Given in facilities or homes for people who have about six months or less to live. Provide physical and emotional care and comfort. Activities of daily living (ADL's) - Answer-Bathing, skin care, nail and hair care, assistance with walking, eating, dressing, transferring, and toileting. Culture change - Answer-Term given to the process of transforming services for elders so that they are based on the values and practices of the person receiving care. Involves respecting both elders and those working with them. Person-directed care - Answer-Emphasizes the individuality of the person who needs care, and seeks to build community by recognizing and developing each persons capabilities. Promoting dignity and providing caring environments is key. Medicare - Answer-A federal health insurance program that was establish in 1965 for people aged 65 or older. It also covers people of any age with permanent kidney failure or certain disabilities. Medicaid - Answer-A medical assistance program for low-income people, as well as for people with disabilities. It is funded by both the federal government and each state. NA - Answer-Preforms assigned tasks, such as taking vital signs. Also provides or assists with person care, such as bathing residents and helping with toileting.RN - Answer-Coordinates, manages, and provides skilled nursing care. Includes giving special treatments and medications as prescribed. Also assigns tasks and supervises daily care of residents by NA's. LVN - Answer-Gives medications and treatments. Also supervise nursing assistants daily care of residents. Doctor - Answer-Diagnoses disease or disability and prescribes treatment. PT - Answer-Evaluates a person and develops a treatment plan. Goals are to increase movement, improve circulation, promote healing, reduce pain, prevent disability, and regain or maintain mobility. Gives therapy in the form of heat, cold, massage, ultrasound, electrical stimulation, and exercise to muscles, bones, and joints. OT - Answer-Helps residents learn to adapt to disabilities. May help train residents to perform daily living. Speech-language pathologist - Answer-Identifies communication disorders, addresses factors involved in recovery, and develops a plan of care to meet recovery goals. Teaches exercises to help the resident improve or overcome speech problems.
