These medications bind to the beta-adrenoreceptor blocking the binding of norepinephrine and epinephrine which inhibits sympathetic effects. Cardiac index - ANS-normal: 2.5 - 5 L/min Pulmonary vascular resistance (PVR) - ANS-measures afterload of the RIGHT heart Normal: 50-250 dynes Increase in PVR - ANS-caused by acidosis, hypercapnia, hypoxia, actelectasis, ARDS Decrease in PVR - ANS-caused by alkalosis, hypocapnia, vasodilating drugs Systemic vascular resistance (SVR) - ANS-Normal: 800-1200 dynes Increase in SVR - ANS-Can be caused by vasoconstriction due to hypothermia, decreased cardiac output, hypovolemic shock, aftermath of pressors Decrease in SVR - ANS-Can be caused by vasodilation due to anaphylaxis, neurogenic shock or septic shock, vasodilating drugs like nitroprusside, nitroglycerine, hydralazine. Heart sounds - ANS-Due to valves opening and closing. *"Toilet paper my arse" - tricuspid, pulmonic, mitral, aortic S3 - ANS-"Ken-tuck- ee" Heart sound that can be due to fluid overload. Usually occurs due to overfilling of the left ventricle, CHF, choridae tendonae dysfunction S4 - ANS-"Tenn-ee-ssee" Usually due to a weak left ventricle, cardiomyopathy, hypertension or infarction (MI). You would expect to hear this heart tone from a patient with a history of an old MI How do you treat hyperkalemia? - ANS-IV insulin, D50, Kayexelate * if your monitor is double counting your QRS or T waves it may indicate hyperkalemia and you may need to administer calcium Hypocalcemia - ANS-with this electrolyte imbalance you will experience decreased contractility and increased irritability Hypercalcemia - ANS-with this electrolyte imbalance you will experience increased contraction Hypomagnesemia - ANS-with this electrolyte imbalance you will have decreased conduction. Hypermagnesemia - ANS-with this electrolyte imbalance you will experience increased myocardial irritability and decrease DTRs (reflexes) Cardiac injury management - ANS-aspirin oxygen - don't want to put them on NRB @ 15 lpm because causes hyperoxia, free radicals and vasoconstriction. Only apply as needed; if under 94%. nitroglycerin - dilates coronary arteries and decreases afterload / SVR betablockers - decreases oxygen demand and increases ejection fraction, helps to decrease heart rate plavix/heparin - anticoagulants (LMWH - lovenox 60 units/kg or 12 units/hr) integrellin/Agrastat - they get this on the way to the cath lab. Has a shorter half life than aspirin but essentially does the same thing. Fibrinolytics - ANS-Used in cardiac injury management. Contraindications include any non-compressible bleed or neoplasm indications include acute myocardial infarction within 12 hours, EKG showing STEMI in two or more leads, new onset of LBBB Reperfusion - ANS-in cardiac injury management these patients will have a sheath from the cath lab. In a CABG they use the saphenous vein for the RCA and for the LAD they use the inferior mammary artery which is usually associated with a hemothorax so keep an eye out for this. 120 - 22- joules - ANS-how much energy you use for defibrillation of an adult in ventricular fibrillation or ventricular tachycardia Dopamine renal dose - ANS-1-2 mcg/kg/min no cardiac effects with this dose. Dopamine beta 1 dose - ANS-5-10 mcg/kg/min beta 1 effects (cardiac); helps to increase heartrate and contractility (chronotropy and inotropy) Dopamine dose for shock - ANS-15-20 mcg/kg/min used for distributive shock and hypotension because increases SVR
