I completely see how you have a problem with documenting by exception. I am a brand new nurse, & I have the "luxury", if you can call it that, of being required to narrative chart on paper. I feel like this is a blessing in some ways. I am almost hyper-aware of everything I do & how I document. Some entries from others are so brief & vague, I don't feel like I have a clear picture of my patients sometimes. During nursing school, I did do a rotation in which everything was documented by exception, & while there was an option to add a comment, I noticed that many nurses did not. I tried to add as much detail as I could. I do feel this way may be quicker, but the true view of the patient as an individual can be lost...A sort of "cookie cutter" method of charting. I have heard experienced nurses voice that the "art" of nursing has been lost.
