P5: Explain the benefits of promoting personalisation when overcoming challenges faced
by individuals with different needs.

Personalisation is a term used to describe the process of ensuring that each person
receiving assistance and support is able to set their own personal goals and has control and
choice over the shape of their care and support. Promoting choice allows service users to
exert their individual rights to the greatest extent possible. Be self-sufficient, make their
own decisions, and feel in command (docs.google.com).

A care needs assessment from the local council can be the first step towards obtaining the
assistance and support users require in their daily life. The goal is to determine how much
assistance users require to live as independently as possible (moneyhelper.org.uk). The
national eligibility criteria; that is, whether their needs are the result of or are related to a
mental or physical disability or illness, Make it impossible for them to achieve two or more
specified outcomes. Outcomes such as receiving financial support, being admitted to a care
home or being provided with information and advice. Meaning that failing to meet these
two or more outcomes is likely to have a significant impact on their well-being

Care planning is a discussion between a person and a healthcare practitioner about the
impact their condition has on their life and how they can be best supported to meet their
health and wellbeing needs throughout their lives. The individual owns the care plan, which
is shared with others with their permission. It is critical that a discussion takes place, that a
record of it is kept, and that everyone is aware of the plan (scie.org.uk). The care plan
ensures that clients have the right to be self-sufficient, to make their own decisions and
choices, and to feel in control of their lives. It Encourages choice and control in order to
assist clients in maintaining their independence and positive self-esteem. Plans are
classified into three types: The first is care, which plans the care services and support to be
provided. The care plan can assist individuals who require extra assistance with daily living.
The second plan is Learning; this is a learning programme that takes into account a
person's strengths and weaknesses; the plan assists individuals with learning disabilities.
The third plan is the behavioural plan, which explains how a person's behaviour can be
changed. The behavioural plan assists people with conditions such as autism

Promoting choice and control is critical because it ensures clients' right to be independent,
to make their own decisions and choices, and to feel in control of their lives, as Billy may
feel patronised as he may feel as if his social worker is trying to control his life, which may
have a negative impact on his mental health and It encourages Billy to exercise choice and
