It happens daily, patients being pushed through the emergency department to their assigned inpatient bed to accommodate the continuous flow of incoming ED patients. A quick two-to-three-minute telephone report is given to the receiving nurse and the patient is transferred out of the ED. How much information was given, and how accurate was it? Hand-off report is the transfer of information from one nurse to another nurse that will be assuming care of the patient. Chladek et al., (2021), reports that miscommunication and omission of information during patient handoff reports are one of the leading causes of medical errors. A thorough transfer of information is necessary to ensure continuity of care and to prevent adverse events and medical errors. Therefore, it is proposed that inpatient nurses receive bedside report in the ED using a designated checklist designed to improve patient hand-off for ED patients being admitted to inpatient units.
