,Tasks and lectures
Task 1 / Lecture 1 – Introducing something new
Innovation
Rogers (2003): “An innovation is an idea, practice, or object that is perceived as new by an
individual or other unit of adoption. It matters little, so far as human behaviour is concerned,
whether or not an idea is objectively new as measured by the lapse of time since its first use
or discovery.”
- Newness in an innovation need not just involve new knowledge; may be expressed in
terms of knowledge, persuasion, or a decision to adopt.
- It should not be assumed that the diffusion and adoption of all innovations are
necessarily desirable.
- This definition is useful for individual behaviour, but less useful for organisational
level
- Organisational innovation precipitates some kind of planned change in the structures
and systems in the organisation
- Taking into account all the differen types Greenhalgh formulated a new definition;
Rogers (2003) – Technological innovations: a design for instrumental action that reduces the
uncertainty in the cause-effect relationships involved in achieving a desired outcome.
- Hardware aspect → consisting of the tool that embodies the technology as material
or physical objects.
- Software aspect → consisting of the information base for the tool.
o Computer hardware or computer software.
Technological innovation creates one kind of uncertainty in the minds of potential adopters
(about its expected consequences), as well as representing an opportunity for reduced
uncertainty in another sense (that of the information base of the technology).
1. Software information, which is embodied in a technology and serves to reduce
uncertainty about the cause-effect relationships involved in achieving a desired
outcome.
2. Innovation-evaluation information, which is the reduction in uncertainty about an
innovation’s expected consequences.
Osbourne: Key elements
1. Innovation represents newness
2. It is not the same thing as invention
o The latter is concerned with the discovery of new ideas or approacheswheras
innovation is concerned with their application
3. It is both a process and an outcome
o It is more than just an idea (invention is just the idea)
o It is strongly linked to you just adopt an innovation
▪ The users need to adopt this innovation
▪ The innovation can focus on the process; creating new logistics to look
at a process
4. It involves discontinuous change (=sudden shift)
o Innovation is concerned with mostly the application
o Innovation is concerned with the discovery of new ideas or apporaches
whereas innovation is concerned with their application

, Greenhalgh (2005): “An innovation in health service delivery and organisation is a set of
behaviours, routines, and ways of working, along with any associated administrative
technologies and systems, which are
- Perceived as new by a proportion of key stakeholders
- Linked to the provision or support of healthcare
- Discontinuous with previous practice
- Implemented by means of planned and coordinated action by individuals, teams or
organisations
- Directed at improving health outcomes, administrative efficiency, cost-effectiveness
or the user experience
Innovation = the implementation of an internally generated or a borrowed idea – whether
pertaining to a product, device, system, process, policy, program or service – that was new to
the organization at the time of adoption. Innovation is a practice, distinguished from
invention by its readiness for mass consumption and from other practices by its novelty
- Health care innovation = a medical technology, structure, administrative system, or
service that is relatively new to the overall industry and newly adopted by hospitals in
a particular market area.
- Service innovations = innovations that incorporate changes in the technology, design,
or delivery of a particular service or bundle of services
- An innovation in health service delivery and organisation is a set of behaviours,
routines and ways of working, along with any associated administrative technologies
and systems, which are:
Grol (2013): A new procedure or technique, a guideline, a care protocol of a ‘best practice’
----
Different types of innovations:
- Product innovations: “Introduction of a good or service that is new or significantly
improved with respect to its characteristics or intended uses. This includes significant
improvements in technical specifications, components and material, incorporated
software, user friendliness, or other functional characteristics”.
o Supportive technology like electronic drug dispensers
o Interventions to stimulate PA (walking groups)
o Therapeutic interventions like Paro
- Process innovations: “Implementation of a new or significantly improved production
or delivery method. This includes significant changes in techniques, equipment
and/or software”.
o Electronic patient records
o Care at distance
o Digital planning an registration systems
- Organisational innovations: “Implementation of a new organizational method in
firm’s business practices, workplace organization or external relations”
o E-learning
o Self-management training for staff
- Marketing innovations: “Implementation of a new marketing method involving
significant changes in product design or packing, product placement, product
promotion and pricing”
- Combinations of different types of innovations

Vaak complexer dan nodig is, helft in het Engels en half in het Nederlands. Maar essentiëler: case 6 mist!
