Student Response: I utilized the SOAP note format to help guide my interview and questioning of Tina during
her office visit. When answers to questions provoked further questioning I presented these to Tina in order to

better understand the underlying circumstances of her presenting symptoms. I took opportunities to empathize
and educate Tina on certain aspects that were relevant and had presented during the interview. I utilized my

presenting symptoms and how best to treat them.

Explain the clinical reasoning behind your decisions and tasks.
Self-Reflection
Student Response: The clinical reasoning associated with my decisions and the tasks that I undertook to
complete these had to due with the patients presenting symptoms and her reports of what led up to the
causation of the symptoms. The patient was in no acute distress, so I was able to fully interview and exam her
for not only the subjective information but also for the obervational and diagnostic information that was aquired
in order to determine probable diagnoses along with appropriate courses of treatment. I was able to provide
education on the diagnoses along with their respective treatments so that the patient was well educated and
therefore could better understand the need for compliance in managing her health.
Identify how your performance could be improved and how you can apply “lessons
learned” within the assignment to your professional practice.
Student Response: I feel that I did somewhat better on this assignment in comparison to the previous
assignments, but there is always room for improvement! I know that I need to be quicker with my
documentation and that I need to improve on my interviewing skills. I also could use some further improvement
on my physical examination skills, but feel that these will all improve with time and practice. I am always
looking and asking my peers for advice and tid-bits on how to better these aspects. I also had incorporated
advice from my professor in this shadow health assignment that seemed to improve my organization and flow.
