● Brain metastasis complicates systemic malignancies in 30% of cancer patients.

○ niet zeldzaam: life time risico van mensen met kankerdiagnoses: 30%
○ subklinisch: niet alle mensen komen hiermee in de kliniek
■ indolent en beperkt
■ laat in ziekteverloop wanneer andere symptomen van de aandoening de
symptomen van de maligniteit overtreft
○ nooit puur lokaal - altijd ergens een regio van oorsprong in een ander orgaan

, 1. Single brain metastasis AND Significant mass effect (symptoms !) or no pathologic
confirmation of the primary: surgical resection if feasible
○ je weet nog niet zeker welk primair gezwel de metastase veroorzaakt, omdat er
geen weefselbewijs
■ bv: patiënt heeft een ringvormige laesie in het brein.
■ CT thorax toont misschien een verdachte longnodulus, maar niet biopt-baar.
■ De hersenlaesie geeft een mass effect of symptomen.
■ Dan ga je resecteren omdat: je tegelijk decompressie doet én het stuk tumor
nodig hebt voor pathologie ⇒ systeemtherapie bepalen
○ → vroeger gingen ze enkel eenvoudige metastasen opereren
■ statistisch geassocieerd met grotere kans op unieke ervaring
■ wss nergens anders micro-metastasen
○ nu: niet meer helemaal juist
■ zelden of nooit mensen opereren met meerdere metastasen
● je bent te laat, je loopt achter de feiten aan
● er zijn er wss meer ⇒ gaan weer vermenigvuldigen
■ ↔ symptomen: couranter, minder streng
● ⇒ heelkunde gaat pt symptomatisch verbeteren
2. Single brain metastasis, asymptomatic: therapy depends on
○ Extent and activity of the extracranial disease
○ General medical condition and performance status = systemische ziekteload
■ → enkel wnr alles buiten de hersenen stabiel is: lokale therapie zoals
chirurgie
○ Patient's preference
3. Stable or absent extracranial disease:
○ Better OS and freedom from neurologic progression if surgery preceeds WBRT
shown in 2 RCT
■ Bij patiënten met een enkelvoudige hersenmetastase en stabiele of geen
extracraniële ziekte is er bewijs (2 RCT’s) dat eerst opereren en pas daarna
whole-brain radiotherapie (WBRT) leidt tot:
● betere overleving
● langere periode zonder neurologische progressie
● betere lokale ziektecontrole
■ Dit sluit aan bij de uitleg van de prof: chirurgie geeft snelle cytoreductie
(grote tumorlast in één keer weg), en vooral bij een solitaire metastase met
lage systemische tumorload levert dat oncologisch voordeel op.
○ No advantage of surgery in OS and QOL seen in I RCT (more EC disease however in
this trial)
■ Eén studie vond geen verschil in overleving of levenskwaliteit tussen
chirurgie+WBRT versus WBRT alleen.
■ De prof vermeldt waarom: die studie had slechtere patiënten, namelijk veel
extracraniële ziekte.
■ Bij wie de ziekte buiten de hersenen toch actief is, heeft een lokale
hersenoperatie weinig impact op globale overleving, omdat de systemische
progressie het verloop bepaalt ⇒ de systemische ziekte load is een kritieke
selectiefactor voor chirurgie.
4. very radiosensitive metas (SCLC, leukemia, lymphoma, germ cell tumor): WBRT alone
○ indien primaire tumor goed reageert op radiotherapie ⇒ uitsluiten chirurgie
