Pathogenese Commensalen bovenste luchtwegen → aspiratie

Diagnostisch Sputumcultuur (etterig fluim), hemocultuur, gramkleuring, Cave: is commensaal!
Epidemiologie Weinig seizoensvariatie, gestegen resistentie (20% heeft β-lactamase)

Klinisch Tracheobronchitis, milde bovenste luchtweginfecties & atypische pneumonie.
(Vooral bij jongeren, hoofdpijn, bilaterale longaantasting, niet productieve hoest, faryngitis &
koorts)
Diagnostisch Te klein om microscopisch te zien, trage cultuur, via antistoffen of PCR
Behandeling Antibiotica (Azitromycine) versnelt genezing maar wel gewoon nog transmissie

Behandeling Tetracyclines of macroliden, vaak herbehandeling nodig
Legionella pneumophila: gram- bacillus
Virulentie Intracellulaire overleving (geen fusie fago-/lysosoom), vernietiging macrofagen mbv
proteolytische enzymes, biofilms. Legionella gaat pas dood als macrofaag activeert.
Pathogenese Dropletinfectie (airco!) → adhesie aan de alveolaire macrofagen via C3b
Klinisch Atypische pneumonie, hoofdpijn, niet productieve hoest, GI last, verward, thoracale pijn, vooral

Epidemiologie Asymptomatisch, heel jaar door, epidemieën in de herfst
Diagnostisch Cultuur van tracheaal aspiraat, serologie, PCR, of Ag test van de urine
Behandeling Macroliden + preventief hittebehandeling van watercircuits
Mycobacterium tuberculosis: gram- bacillus, zuurvast
Virulentie Groei op wateroppervlakten, intracellulaire overleving, dikke celwand, ontregeling coördinatie
immuunantwoord, catalase productie
Pathogenese Dropletinfectie → fagocytose door alveolaire macrofagen → intracellulaire vermenigvuldiging in
niet-geactiveerde macrofagen (maturatie wordt geïnhibeerd door inhibitie sfingosine kinase) →
cellyse → nieuwe cyclus → spreiding naar lymfeklieren → activatie T-cellen → vorming
antilichamen, TH & TC → activatie macrofagen → versmelten tot meerkernige reuscellen
(Langhanscellen)/tbc granuloom.
Latente tbc: granulomas omgeven door fibrinekapsel, niet besmettelijk.
Actieve tbc: granulomas verkazen → luchtwegaantasting → vorming cavernes → destructie
longweefsel → uitzaaiing & etterige hoest.
Klinisch Longtuberculose (afvallen, malaise, nachtzweten, sputumproductie)
Extrapulmonale tbc (vanuit de longen naar andere lichaamsdelen)
Epidemiologie Wereldschaal, vooral lage SES (SSA) & laag immuunsysteem (HIV/AIDS), door contact met tbc
patiënt, door toegenomen mobiliteit ook in België (1/10.000 per jaar)
Diagnostiek Z-N of Auramine kleuring fluim, cultuur van 2ml fluim (decontaminatie!), PCR 100 μl fluim
Behandeling 1e lijn: rifampicine (+/- isoniazide) → 10% is rifampicine-resistent (waarschijnlijk ook MDR).
2e lijn: fluoroquinolonen of tuberculostatica → 5-10% zijn XDR
Preventie Tuberculinetest, BCG-vaccinatie (in UK), DOTS
