#1: How has your understanding changed about identifying the
most commonly prescribed agents in the major drug classes? I
believe my understanding in identifying the most commonly prescribed
agents in the major drug classes has definitely improved, however, I still
have much to learn as medication prescribing can be very challenging. I have
come to realize that there are so many factors to consider before prescribing
even the most basic medications. The reading assignments and projects in
this course have helped me to better understand the commonly prescribed
medications that are in the major drug classes for common diagnoses, and I
am relieved that there are many resources I can utilize to assist me with
understanding the mechanism of action, therapeutic indication, and side
effects for commonly used medications. Having resources available to me
makes me feel much better and not so alone in my decision-making process.
#2: How confident do you feel about analyzing factors pertinent to
making the most effective drug selection for a patient with a
specific diagnosis in the primary care setting? What resources are
needed, if any, to help you feel more confident? As I mentioned
previously, I feel I have learned a lot thus far, but to say that I am confident
is somewhat of a stretch. I am not sure how I am supposed to feel at this
stage in my learning, but I feel I have much more to learn before I can feel
100% confident about analyzing factors that are pertinent to making the best
medication selection for my patients. There are so many factors to consider
when deciding on which medication to prescribe. With each course in this
program, I feel my confidence has increased, but I realize that primary care
deals with the entire body, so my patient can walk in with a multitude of
problems, and it will be my sole responsibility to treat them, which is very
intimidating. With that said, I realize that I must assess all body systems
before any decisions can be made. For example, if my patient walks in with a
respiratory issue, I need to perform a comprehensive exam and not just
assess the lungs, before I can prescribe medication. Providing excellent and
safe care will require me to utilize many resources that are available to
providers. Epocrates, Medscape and Prescribers Digital References are some
of the resources that can help me when deciding on the proper drug
selection for my patients. In addition, there are many organizations that exist
such as the American Association of Nurse Practitioners (AANP) which will
help keep me up to date on the current trends and information and changes,
etc. There are many associations such as the American Diabetes Association
and the American Heart Association, which are excellent resources,
especially the Clinical Practice Guidelines, which will guide me in my
decision-making process and help me increase my knowledge base and
confidence.
