Degree/Education Required - ANSGraduated and received a degree from an ACPE
approved school or college of pharmacy.
If foreign Graduate they must complete the FPGEC certificate requirements. (Foreign
Pharmacy Graduate Equivalency Program Certificate)
Guidelines of license reinstatement? Less than four years. - ANS1. Submit written
request for reinstatement.
2. Pay all back renewal and penalty fees.
3. Submit CV going back to date of expiration.
4. Complete and obtain proof of 30 CE completed in the past two years.
5. If licensed in another state have verification sent to the board of pharmacy.
If administratively lapsed board may also require to:
1. Applicant complete CE pool for current cycle
2. Meet with the board.
Guidelines of license reinstatement? Longer than four years. - ANS1. Completed
application for reinstatement
2. Re-take and achieve passing MPJE score
3. Complete and submit proof of 30 hours CE
4. Payback renewal and or penalty fees.
-If out of practice for four years must get 1000 hours as intern again. Each year out of
practice over 4 years is an extra 100 hours up to 14 or more years cap 2000
How are board members elected? - ANSThey are appointed by the governor.
How long do they serve for? - ANSFive years or until their successor is appointed.
How many CE hours are required for license renewal? - ANS30 hours or 3.0 CEU
