Client Profile • Gender: female • Age: 27 yo • Weight: 130 lbs • NKDA • Setting: hospital • Ethnicity: white American • Cultural Considerations: none identified • Past Medical History: several Urinary Tract Infections (UTI) over the past three years ago • Communication: English • Disability: none • Social: married; stay at home mom; one daughter (9 mos); husband works 2 jobs Chief Complaint / History of Present Illness Ms. Kellar c/o sudden onset abdominal pain approximately 12 hours ago. She describes the pain as a constant ache in the lower right quadrant (RLQ) of her abdomen that radiates across the flank to her lower back. She rates her pain as a 9/10. She also c/o nausea and has vomited three times prior to arrival. She is febrile and c/o chills. During your initial history, Ms. Kellar tells you that she has had urinary frequency with “darkish funny smelling urine for a few days.” She also recalls a lower back ache for several days and feeling “hot and cold.”
