The establishment of health promotion and personal well-being begins at birth spans the continuum of a person’s life. Defining factors of personal health reside in the social determinants of health, including health and healthcare, education, economic stability, social and community context, and neighborhood and built environment (U.S. Department of Health and Human Services, Office of Disease Prevention and Health Promotion, 2020). To assess the social determinants of health, a comprehensive health assessment of the patient is paramount in establishing the current status, and devise the plan of care to assist the patient in maximizing health and well-being.
