What would the RN tell a patient who calls complaining of contractions. She cannot
recall the duration or frequency but states they are not progressing in intensity and
she's feeling discomfort in her groin. Ans: Patient is likely experiencing false labor.
Contractions should become increasingly more intense, closer together, and longer.
Walking or exercise will increase contractions. Pain is felt in the lower back and
sweeps around the girdle.

A patient comes in suspected to be in labor. How does the RN KNOW that it is true
labor? Ans: Cervical effacement and dilation will occur and progress typically in an
observation period of 1-2hrs. (cervical exam)

What are the power's of labor? Ans: Maternal pushing efforts and uterine contractions

What is important to note about the pelvis in the context of giving birth? Ans: Relaxin
may allow slight relaxation of the pelvis but the bone does not readily give to the
forces of birth so pelvic shape is important.

What position is ideal for laboring? Ans: Cephalic, specifically, occipital anterior
position (face down, head up)

The RN is educating a patient that their baby will have two "soft spots" or fontanels on
their head. The patient ask what the purpose of the soft spots are. What is the best
response? Ans: The fontanels allow molding of your infants head so they can fit
through your pelvis.

The HCP does an ultrasound of your laboring patient. The fetus is coming straight

What is normal fetal heart tone? Ans: Regular rhythm, with accelerations from
baseline in response to stimuli

Describe early decelerations. Ans: Occur during contractions and are normal during
the second stage of labor . Commonly caused by fetal head compression against the
pelvis or soft tissue.

Describe late decelerations. Ans: Occurring between maternal contractions

Describe variable decelerations? Ans: Vary between and during contractions

What is the RNs response if variable decelerations are present? Ans: Stop oxytocin if
present, administer tocolytic, reposition the mother to left side lying position, increase
IV fluid rate, notify HCP, and administer O2 at 8-10L

How would the RN describe Variability of <5bpm Ans: minimal

How would the RN describe Variability of 6-25bpm Ans: moderate

How would the RN describe Variability of >25bpm Ans: marked

What is the common cause of variable decelerations Ans: Cord compression
