Diagnosis of Neurodevelopmental and/or Disruptive Impulse-Control and Conduct
Disorders
Diagnosis
F91.3: Oppositional defiant disorder – moderate

Pattern of angry/irritable mood, defiant behavior, and vindictiveness.

Code- Z62 – problems with upbringing

Justification of The Diagnosis
The DSM–5- TR specifies that an individual with this disorder demonstrates three

categories of symptoms: angry/ irritable mood, argumentative/defiant behavior, and vindictiveness

lasting at least 6 months. The individual shows at least four symptoms from each of the three

categories of symptoms (American Psychiatric Association, 2022). Aponi demonstrates symptoms

of angry /irritable mood, as she is often angry and easily loses her temper. She always throws

tantrums when she feels she does not get enough of what she wants. Her tantrums are usually

characterized by angry and irritable behaviors, including shouting, cursing, crying, slamming

doors, and sometimes throwing things.

She also demonstrates defiant behavior by intentionally annoying other people. She

deliberately causes trouble in school by bullying other students and taking their things away. She

also has a habit of provoking other students and attempting to initiate fights, which fits with the

DSM-5 - TR, which describes defiant behavior as "often deliberately annoying others." She blames

others for her mistakes. For example, when her mother confronts her for continuously kicking the

chair, she gets annoyed, stating that she is not doing anything wrong and her mother is picking on

her. The DSM5- 5- TR criteria describe vindictiveness as demonstrating distress and distressing

others in the immediate social context (American Psychiatric Association, 2022). Aponi is bullying

her peers in school, provoking them, annoying them, and making them feel distressed. Her teacher

has expressed how she always creates trouble with her classmates.
